Building a successful private practice in the UK is a dream for many physiotherapists, osteopaths, and healthcare professionals. However, the path to a thriving clinic is rarely the smooth, upward trajectory that social media might lead you to believe. When we look at successful clinic owners, it is easy to assume they moved from strength to strength without a single stumble. The reality is far more complex.
Success in the healthcare business is more like a roller coaster at Blackpool Pleasure Beach than a steady climb. There are peaks, sudden drops, and sharp turns that can leave even the most seasoned practitioner feeling slightly dizzy. Having opened my first clinic back in 2015, I have experienced these highs and lows firsthand. If I were starting out from scratch today, there are five fundamental things I would do differently to navigate that roller coaster with more confidence and less stress.
1. Get Crystal Clear on Your Why and Your Values
When you first open your doors, it is common to adopt an all-hands on deck approach. You are the clinician, the receptionist, the cleaner, and the bookkeeper all rolled into one. In this whirlwind of activity, it is easy to lose sight of why you started the business in the first place.
If I were starting again, I would spend significantly more time defining my "why" before seeing my first patient. Your business values must be an extension of your personal values. If you value freedom and flexibility but build a business model that requires you to be at the treatment table 60 hours a week, you will eventually face burnout and resentment.
Alignment is the key to sustainability. When your business decisions are filtered through your core values, you create an organisation that feels authentic to you and attractive to your ideal patients. Take the time to write down what you stand for. Is it clinical excellence? Is it community? Is it innovation? Once these values are set, every hire you make and every service you launch should align with them.
2. Listen to One Person and Turn Off the Noise
We live in an age of information overload. As a clinic owner, you are constantly bombarded with advice on Facebook, Instagram, and LinkedIn. You see what other clinics are doing and immediately feel the urge to copy them. This comparison is the biggest thief of joy and the fastest way to lose your focus.
If I could go back to 2015, I would choose one mentor or one specific methodology to follow and I would ignore everything else for at least twelve months. When you try to follow five different business coaches at once, you end up with a fragmented strategy that leads nowhere.
Focus on your own lane. Comparison often leads to making decisions based on someone else's highlight reel rather than your own business data. By choosing one trusted source of truth, you reduce decision fatigue and give your strategy the time it needs to actually work.
3. Understand Your Team Dynamics Through the Working Geniuses
In the early days, I believed that if I just hired good people, the team would figure itself out. I quickly realised that even a group of talented individuals can struggle if their roles do not match their natural strengths.
I highly recommend exploring the Six Working Geniuses by Patrick Lencioni. This framework helps you understand where you and your team members thrive. For instance, are you a "Galvaniser" who loves to get people moving? Or are you a "Founder" who excels at the initial spark of an idea? Perhaps you have the gift of "Tenacity," the ability to see a project through to the very end.
If I were starting today, I would use these profiles to ensure that every person is in the right seat. Many clinic owners struggle because they are trying to do tasks that drain their energy. If you hate the minute details of health insurance billing but have a high genius for "Enablement," you are likely holding your business back by not delegating the technical tasks to someone with a genius for "Tenacity." Recognising these traits helps remove the metaphorical monkey from your shoulder and allows your team to flourish.
4. Ask for Help Sooner
There is a common misconception that being a business owner means having all the answers. In my first few years, I thought asking for help was a sign of weakness or an admission that I was failing. I now know that asking for help is actually a sign of leadership and an abundance mindset.
Making decisions from a place of abundance means believing there is enough success to go around. It means being inspired by others rather than intimidated by them. If you are struggling with your clinic's finances, your marketing, or your team management, do not wait until you are in a crisis to seek help.
Whether it is a business coach, an accountant, or a peer support group, surrounding yourself with people who have already achieved what you want to achieve is the fastest way to grow. You do not have to figure everything out by yourself through trial and error.
5. Upgrade Your Environment and Your Mindset
You have likely heard the saying that you are the average of the five people you spend the most time with. This is incredibly true for clinic owners. If you spend all your time with people who complain about the NHS, the economy, or the difficulty of finding staff, your own mindset will begin to reflect that negativity.
If I were starting out again, I would be much more intentional about my inner circle. You need to find people who challenge you and who have a higher "thermostat" than you do. If your internal thermostat is set to a certain level of income or a certain size of clinic, you will subconsciously stop yourself from growing beyond that point.
By surrounding yourself with people who operate at a higher level, you naturally begin to upgrade your own expectations. This is not about being elitist; it is about protecting your energy and ensuring you are constantly being pulled toward growth rather than pushed down by mediocrity.
Conclusion
Starting and growing a clinic in the UK healthcare landscape is a challenge, but it is also one of the most rewarding things you will ever do. By getting clear on your values, narrowing your focus, understanding your team's natural geniuses, asking for help, and upgrading your environment, you can build a business that serves both you and your patients.
Remember, success is not a straight line. Embrace the twists and turns of the roller coaster, learn from the drops, and keep your eyes on the long term vision of what you want to create.
